Individual Gift Membership Application - for Someone You Know

Individual Renewal Membership Application - for Renewing Members

Non-Profit Association / Club Membership

Association or Club with fewer than 25 members:

$100/Yr.

Association or Club with more than 25 members:

$4/Member/Yr. not to exceed $1000/Yr.

NOTE: Payment of fees (and/or insurance premium) must be made before LTNA membership or insurance coverage is activated.

Step 3: If interested, contact Executive Director Katie Dejonge (1.877.858.8627) to inquire about the cost of optional liability insurance for your non-profit organization before you submit your application; This insurance is NOT AVAILABLE to individual or corporate/institutional/agency members;

Step 4: If submitting an application for an association, club or corporate membership, return your completed application and membership fee payment check.
